ActionADE is a web-based application developed, piloted, refined and implemented by our team that allows providers to document and communicate standardized adverse drug event information to providers in other health settings to prevent unintentional re-exposures to harmful medications.
Our research team focuses on designing, implementing, and evaluating innovative adverse drug event interventions in health care, such as ActionADE. We also focus on identifying effective approaches to scale-up, spread and sustain such interventions. We believe that scaling up evidence-based adverse drug event interventions, provincially and/or nationally will improve medication safety for all Canadians.
“As the Clinical Pharmacy Coordinator at Richmond Hospital, I think it’s an important activity for pharmacists to identify and document Adverse Drug Events as per Vanessa’s Law and ActionADE has provided the platform to do that. Since I also practice clinically in critical care, one of the questions I always have when I review my patients is whether their admission is medication-related. As a result, I’ve been able to integrate ActionADE in the daily assessment and follow-up of my patients and I try to report in ActionADE at the end of the working day. I’m a big advocate of seamless transmission of information throughout the interfaces of healthcare. ActionADE gives me an opportunity to notify my pharmacy colleagues in community and primary care about ADEs that have led to hospitalization in the patients I’m following.”
“ActionADE serves as more than just a platform; it acts as a catalyst for positive change within our healthcare system, promoting improved communication, informed decision-making, and ultimately, better care and outcomes for patients. As a healthcare professional committed to improving patient outcomes, I advocate and support the adoption and utilization of ActionADE into healthcare practices as it promotes necessary health information transfer and communication. ActionADE’s platform stands as a pivotal advancement in bridging the gaps between care transitions and healthcare settings, markedly improving access to important health information.
One area where ActionADE excels is in addressing adverse drug events, which have a significant impact on patient safety and drug therapy. From my experience, the ability to efficiently report and document emergency department visits resulting from adverse drug events is essential. This not only adds to our collective knowledge base but also helps colleagues in community settings to make more informed decisions about patient care. Indeed, the value of a more seamless transfer of information and communication cannot be overstated.
ActionADE adds an extra layer of patient safety. ActionADE facilitates meaningful interventions such as identifying culprit drugs, adjusting medication doses, and implementing more vigilant monitoring plans. Moreover, it fosters necessary discussions between community pharmacists and prescribers, enabling a collaborative approach to reassessing drug therapies and mitigating future risks. These interventions not only improve patient safety but also contribute to a higher standard of care.”
